چکیده

A survey of the discrepant findings regarding the effects of attention focusing and distraction on exposure suggested that subjective measures of anxiety and avoidance respond better to the latter condition, and heart rate (HR) reaction responds to the former. To test this hypothesis, 63 dental phobics were recruited who had not visited a dentist for a mean of 6.6 (1.5-25) years. Participants received a 1-h exposure session with either attention focusing or distraction. Subjective anxiety and HR to phobia-related pictures were assessed before and after the treatment session and again after 1 week. Avoidance was recorded in terms of adherence to the dental treatment schedule in the following 6 months. Contrary to expectation, state anxiety showed a greater decrease in the attention focusing than the distraction condition after 1 week. Both treatment conditions were similarly effective with regard to HR and avoidance. HR habituated in both groups after exposure and 73% of followed-up patients adhered to the dental treatment schedule. Comparison of the present with previous results suggests that the differences between attentional conditions tend to be more pronounced during shorter exposure sessions than were employed in the present study.

Frontal late positivity in dental phobia: a study on gender differences.

Although dental phobia afflicts men and women, gender differences in neural correlates of this disorder have not been investigated thus far. We recorded event-related potential (ERPs) in 30 individuals with dental phobia (15 women, 15 men with comparable disorder severity) and 30 nonphobic controls (15 women, 15 men) while they passively viewed pictures depicting dental treatment, generally fea...

Optimal attentional focus during exposure in specific phobia: a meta-analysis.

Over the last 30 years, researchers have disagreed over the consequences of diverting attention from threat for exposure efficacy, which is an important theoretical and clinical debate. Therefore, the present meta-analysis assessed the efficacy of attentionally focused exposure against distracted and attentionally uninstructed exposure regarding distress, behavioral, and physiological outcomes....

Attentional focus during exposure in spider phobia: the role of schematic versus non-schematic imagery.

Research has provided controversial results regarding the role of distraction (vs. attentional focus) during exposure therapy. In the present study, we manipulated the nature of the concepts activated during exposure. Sixty-six spider phobics were exposed to pictures of spiders and asked, or not, to form mental images of concepts that were either related or unrelated to spiders. At pre-exposure...

Focusing versus distraction and the response to clinical electric shocks.

This study pertains to assessing the effects of electric shocks that are used in the treatment of severe self-injurious behavior. With pain sensation and startle response as the dependent variables and focusing versus distraction of recipient's attention to the electric shocks as the independent variable, these stimuli were administered to 60 paid volunteers. Using ANOVA, no significant effect ...

Efficacy of virtual reality exposure therapy for treatment of dental phobia: a randomized control trial

BACKGROUND Virtual Reality Exposure Therapy (VRET) is found to be a promising and a viable alternative for in vivo exposure in the treatment of specific phobias. However, its usefulness for treating dental phobia is unexplored. The aims of the present study are to determine: (a) the efficacy of VRET versus informational pamphlet (IP) control group in terms of dental trait and state anxiety redu...
